The undersigned hereby makes application for space in the 2009 Builders St. Charles Home Show. Payment (in U.S. Funds) for 50% of booth cost must accompany application contract form. See reverse side for rules & regulations. NO EXHIBIT SPACE WILL BE ASSIGNED UNTIL AFTER AUGUST 1, 2008.

By checking this box and submitting this contract, you agree to the following:

I have read, understand and will abide by all of the provisions of the
Rules, Regulations and Instructions
all of which shall be part of this contract.

Full payment is due February 12, 2009. If full payment is not received before
this date, space shall be forfeited and no exhibit may set-up. No refunds will
be made for booth(s) cancelled after January 14, 2009. Prior to the refund
deadline a 25% of booth cost cancellation fee will apply.

Payment of at least 50% of booth cost MUST accompany this application (unless
invoice option is selected below) & the balance must be paid by February
12, 2009.
Checks payable to the Home Builders Association or HBA. Returned checks will be
charged a $25.00 service fee.
